Please complete all the fields below.  Some device information, such as the Case Number, Product SKU, etc., may be found on the most recent NetAlly Technical Support communication. Once this Shipping Instruction Form has been submitted, NetAlly will deactivate the original device(s) by serial number and re-assign all existing warranty and maintenance services to the replacement device(s) provided. NetAlly reserves the right to provide a replacement device(s) which may not be identical to the original.

PLEASE NOTE: The NetAlly device(s) for which you are submitting a replacement request contains a lithium-ion battery. Per local regulation, such device(s) must be disposed of properly. For more information on the proper disposal within your region please see ….

  • Certificate of Destruction

    By hitting the “Submit” button below, you hereby certify, (i) that you are 18-years of age or older; and (ii) that with respect to this Shipping Instruction Form, you are acting as an authorized representative or agent for an organization or individual which purchased the NetAlly device(s) for which you are submitting this Shipping Instruction Form; and (iii) that you have destroyed or will destroy, at your own expense, the NetAlly devices in accordance with U.S. Environmental Protection Agency Universal Waste Disposal Regulation (U.S. EPA 40 CFR § 273.2 – 273.9, https://www.epa.gov/hw/universal-waste), the European Union Waste Legislation on Batteries and Accumulators (EU Directive 2006/66/EU, https://ec.europa.eu/environment/waste/batteries/legislation.htm), the Australian Battery Recycling Initiative Safety Guidelines (ARBI, https://batteryrecycling.org.au/resource-types/guidelines/ ) and/or any similar regulations applicable to your location, and then-current industry best practices for the disposal of lithium-ion batteries. If you fail to destroy the NetAlly device(s) for which you are submitting this Shipping Information Form in accordance with such regulations and best practices, the organization or individual for which you are submitting this Certificate of Destruction agrees to pay to LinkRunner LLC d/b/a NetAlly the then-current commercial value of the replacement device(s) plus the lesser of one and a half (1.5%) percent per month or the highest rate legally permitted by law.
